Working as a Recovery Coordinator, you will support clients through one-to-one work, care planning, risk assessments, and facilitating multi-agency collaboration. This role involves engaging with clients in individual sessions and group interventions to ensure their overall health and well-being.

Responsibilities include, but are not limited to:

  • Working closely with colleagues, community organisations, and external stakeholders across a range of settings to support clients in maximising their potential.
  • Developing and maintaining relationships with external partners to ensure coordinated and effective client support.
  • Conducting detailed assessments to identify the needs and risks for each client.
  • Developing personalised care plans outlining actionable steps for improvement in the client’s health and well-being.
  • Regularly monitoring and reviewing client progress, adjusting plans to meet evolving needs.
  • Facilitating group interventions that focus on improving psychosocial outcomes, resilience, and well-being.
  • Delivering evidence-based psychosocial programs designed to support long-term recovery and positive client outcomes.
  • Providing integrated support, addressing clients' mental, physical, and emotional needs to promote overall well-being.
  • Collaborating with healthcare and social care providers to ensure that care plans are comprehensive and effective.

Requirements:

  • Passion for working with individuals in recovery and supporting long-term health and well-being.
  • Experience in care planning, risk assessments, and multi-agency collaboration is advantageous.
  • Ability to work effectively within a team and independently.
  • Strong communication skills and ability to engage with diverse individuals and communities.
  • A genuine passion for supporting individuals in their recovery journey.
