We are looking for a Support Coordinator to assist the Senior Management team in the operational management of Birch Lane Project, which supports people with enduring mental health needs within a 24hrs supported living service.

The role involves ensuring that tenants receive person centred support which enables them to enjoy a fulfilling and valued life, to participate in the community and develop their abilities as fully as possible. Provide respectful care, emotional and practical support to people with mental health needs living at the service. You will support and supervise staff, and deploy staff resources effectively to meet the needs of the service. Liaise effectively with involved professionals to provide a consistent and coordinated service.

You will organise and lead 6 monthly person-centred review meetings for the people you are responsible for, in conjunction with their wider care team, ensuring that the service is enabling people to maintain and improve their mental health/physical wellbeing and is developing their capacity for independence. You will ensure that residents enjoy a positive lifestyle, which includes opportunities for personal development and meaningful activity based on their strengths and interests.

When on duty you will take responsibility for leading the shift and deploying support workers, ensuring that the service is well organized and is delivered safely and effectively in accordance with Local Authority standards and agency requirements. You will also take overall responsibility for the service in the absence of the Project Manager carrying out health and safety checks and quality audits. You will provide excellent customer care and proactively communicate with stakeholders in respect of risk management and the care and welfare of the people we support. You will have a strong ethical duty of care and a commitment to safeguarding vulnerable individuals.

Applicants will be able to demonstrate warmth, compassion and unconditional positive regard for the people we support and will be able to develop trusting relationships based on empathy and respect. You will have the skills and personal qualities to be able to engage people in meaningful planning and therapeutic activities. You will be expected to work collaboratively and professionally with the people we support, their families and mental health professionals/agencies.

You will need an up-to-date understanding of the recovery approach and recovery principles as well as mental health legislation, services and interventions. You will demonstrate a good knowledge of the MHA, MCA, DOLs and the care standards. You will have the knowledge and skills to ensure the safe management and administration of medicines. You will be required to work on a rota system.

You will have at least three years’ experience of providing care and support to people with mental health needs.
