Adult's Social WorkerTeam: Hospital DischargeLocation: ManchesterDetails:We are looking to appoint a hospital discharge social worker for a 5 month contract based at Wythenshawe hospital. The role is 35 hours a week, typical hours 9-4.30. Social Worker will primarily be based on site at Wythenshawe hospital, but we can offer up to 2 days hybrid working a week depending on demand. The hospital discharge team, is a friendly supportive team of social workers who work closely supporting discharges from hospital.The candidate will have a firm ethos of a 'home first' approach to hospital social work, and be able to work in a fast paced environment whilst being about to to work flexibly managing competing priorities. The worker must be experienced in completing capacity and best interests assessments. Strengths based assessment skills are essential. The role involves duty work, managing safeguarding's and working dynamically to overcome barriers to discharge. We advise on a home first approach but in situations where this cannot be done the worker will need to complete full assessments for 24hr care placements. Experience in hospital discharge work is essential and the person must be a good team player able to work with others from a variety of disciplines, whilst being compassionate and supporting families.
