Job Description

Service Care Solutions has a new and exciting opportunity which has become available working as a Leaving Care Personal Advisor with Manchester City Council.

Manchester City Council are currently looking for someone who is happy to work 37 hours per week.

The Role

As a Personal Advisor within the Leaving Care Service, you will:

  • Support young people aged 16-25 as they transition from care to independent living.
  • Provide tailored advice, practical assistance, and access to services in education, employment, health, and housing.
  • Work collaboratively with stakeholders to deliver high-quality assessments, develop effective pathway plans, and promote positive outcomes.
  • Manage a caseload, ensuring accurate records, statutory compliance, and holistic care planning.
  • Represent and advocate for young people, ensuring their voices are heard and their needs met.

Key Responsibilities

  • Build and maintain positive relationships with young people, empowering them to achieve stability, safety, and financial independence.
  • Conduct assessments, develop pathway plans, and provide support for education, training, or other needs post-21.
  • Facilitate access to suitable accommodation and financial support while helping young people develop life skills.
  • Coordinate services across statutory and voluntary agencies to ensure seamless, integrated care.
