Job Title: Social Worker - Transition to Adulthood Team
Location: Manchester City Council
Hourly Rate: 30.00 per hour

About the Role:
We are seeking a dedicated and experienced Social Worker to join our Transition to Adulthood Team at Manchester City Council. This role offers a rewarding opportunity to support young people as they transition from children's services to adulthood, ensuring they receive the necessary support and guidance during this critical period.

Key Responsibilities:

  • Conduct comprehensive assessments of young people preparing to transition to adulthood, identifying their needs and developing tailored care plans.
  • Provide direct support and intervention to young people, helping them to access services and resources that promote their independence and well-being.
  • Work collaboratively with young people, their families, and multi-agency professionals to create and implement effective transition plans.
  • Attend and contribute to multi-agency meetings, including transition planning meetings and reviews.
  • Maintain accurate and up-to-date case records in compliance with statutory requirements and organizational policies.
  • Offer guidance and support to young people on a range of issues, including education, employment, housing, and health.

Requirements:

  • Qualified Social Worker with a degree in Social Work or equivalent qualification.
  • Registered with Social Work England.
  • Experience in working with young people, particularly in transition planning within a statutory setting.
  • Strong assessment, analytical, and decision-making skills.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with young people, families, and professionals.
  • Commitment to promoting the independence and well-being of young people.
