If so, we want to hear from you!About the RoleWe are looking for a dedicated and skilled Adults Social Worker to join our Transitions Team within a local East Manchester Council. This role focuses on providing a comprehensive social care service to adults of working age experiencing moderate to severe mental health problems within the community.You will be responsible for:Conducting Care Act assessments and developing clear support plansWorking with young people transitioning from children's to adult mental health servicesCompleting funding applications and legal documentation (CoPDoL, DoLS, 21a's, court reports)Engaging with individuals and families who may be difficult to reachSupporting carers in line with the Care ActCollaborating with multi-disciplinary teams to deliver person-centred careWhat We're Looking ForQualified Social Worker with relevant registration (e.g., Social Work England)Strong experience in mental health social workFamiliarity with Liquid Logic case management systemExperience in transitions work (working with young people moving to adult services)Knowledge of legal frameworks such as DoLS, CoPDoL, and the Care ActWorking Hours & FlexibilityMonday to Friday, 09:00 - 17:00 (some flexibility required for planned interventions)If you're an experienced mental health social worker looking for a rewarding role with a great hourly rate, apply today!Please contact for further information.
