Role:SEND Transition Facilitator
Location:Manchester
Pay rate:£20 - £25 per hour

Duration:1 month
Role responsibilities:
- Works to support the work environment to adapt job requirements to student skills.
- Monitor and problem solve students performance for progress monitoring
- Maintain clear, accurate records of student performance and communicates with students, staff, administrators, and employers
- Maintains employer standards and builds positive, long-term relationships with employers.
- Provides transportation training and assists students in accessing transportation.
- Assists students to acquire independent living skills in the classroom and community.
- Supports integration of assistive technology for students.
- Occasionally provides physical assistance to students with special daily living needs.
- Identify potential students who may need transport and support them with the application process, ensuring that timetables are supplied to the transport team.
- Liaise with parents, carers and potential students to ensure that an individual transition plan is in place where required.
- Attend school EHCP reviews as required.
- Identify current students who may need to support to transition to their next steps, both within college and externally, and work with the SEND team to ensure this is in place.Must be able to start onMonday 17th March 2025covering up to 1 month (possible extension)
- Arrange and host visits to the college for SEND students, in conjunction with the School Liaison team and campus SEND teams.
- 34 hours per week, Monday Friday
- Experience working with students in a mentoring or send facilitator role
- DBS on the update service
- Experiencesupporting students within secondary or a college
- Experience workiing with students with disabilities
