A PCN consisting of 4 practices in the Cambridgeshire area is looking for a clinical pharmacist to join their team.This position is perfect for a clinically minded pharmacist seeking to expand their skill set, as the working week consists of 4 days, on a basis of one day per site (there are 4 practices in this PCN). The ideal candidate would preferably either already be an independent prescriber, or have the desire to work towards this qualification as the client can support you through it.

Key Duties

  • Provide cover across 4 sites of a PCN. 4 day working week (37.5 hours) - 1 day per each site.
  • Work as part of a multi-disciplinary team in a patient-facing role.
  • The post holder will take responsibility for areas of chronic disease management within the practices and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
  • Provide primary support to general practice staff with regards to prescription and medication queries.
  • Support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing.
