As a Social Worker within the Children with Disabilities Team, you will work with children and young people who have a variety of complex needs, including physical and developmental disabilities. Your responsibilities will include assessing their needs, creating individualised care plans, and facilitating access to services that enhance their independence, well-being, and overall quality of life. This role focuses on delivering child-centred support while collaborating closely with families and multi-agency professionals to achieve the best possible outcomes. Additionally, you will ensure compliance with statutory obligations under the Children Act 1989 and other relevant legislation, with a focus on safeguarding and promoting the welfare of children with disabilities.



Responsibilities include:

  • Families Act 2014 to understand the needs, risks, and strengths of children with disabilities.
  • Collaborate with families, health professionals, and education providers to gather holistic information
  • Manage a caseload of children with a range of disabilities, ensuring regular review and adjustment of care plans.
  • Identify and respond to any safeguarding concerns, ensuring that children are protected from harm, abuse, or neglect.
  • Advocate on behalf of children and young people to ensure they have a voice in decisions about their care and support
