Details of the role involve:

Managing a caseload of older adults, often with complex needs such as dementia and/or physical health issues
Completing Care and Support Assessments
Undertaking Care Act assessments
Mental capacity assessments
Best interests decisions
Duty roles in responding to crisis situations
Safeguarding investigations
Some Mental Health discharge work
MH tribunals
Continuing Healthcare work
MDT work
Liaising with other agencies
Planning placements

Requirements of the role:

Qualified Social Worker
Fully Social Work England registered
Must have significant experience in adult social care
Ideal candidate will have expert knowledge of working with older adults in a local authority setting
