Personal Adviser - Pathways Leaving Care Services

Location: Middlesbrough

Position Overview: As a Personal Adviser within our Pathways Leaving Care Services, you will play a pivotal role in providing holistic support and guidance to care leavers as they navigate the challenges of transitioning to independent living. You will work closely with young people aged 16-25, offering practical assistance, emotional support, and advocacy to empower them to achieve their goals and aspirations. This position requires a compassionate and resilient individual who is committed to championing the rights and well-being of care-experienced young people.

Roles and Responsibilities:


  • Build trusting and supportive relationships with care leavers, offering individualised guidance and support to help them develop essential life skills and achieve their personal, educational, and employment goals.
  • Conduct comprehensive assessments of young people's needs, strengths, and aspirations, working collaboratively with them to develop personalised Pathway Plans that reflect their aspirations and aspirations.
  • Provide practical assistance and advocacy to support care leavers in accessing essential services and resources, including housing, education, employment, health care, and financial support.
  • Act as a key point of contact and advocate for care leavers within multi-agency networks, including local authorities, housing providers, educational institutions, employers, and community organisations.
  • Facilitate group workshops, training sessions, and peer support activities to enhance care leaver's skills, confidence, and resilience in navigating independent living and adulthood.
  • Support care leavers in developing essential life skills, such as budgeting, cooking, household management, and accessing community services, through practical guidance and hands-on support.
  • Monitor and review the progress of care leavers against their Pathway Plans, regularly assessing their needs and adjusting support interventions accordingly to promote positive outcomes.
  • A relevant qualification in social work, youth work, or a related field (e.g., Diploma or Degree in Social Work, Youth Work, or equivalent).

    • Previous experience working with care-experienced young people, preferably within leaving care services or a similar context.
    • Excellent communication, interpersonal, and relationship-building skills, with the ability to engage effectively with young people from diverse backgrounds.
    • Strong assessment and casework skills, with the ability to develop and implement person-centred support plans in collaboration with care leavers.
    • Empathy, resilience, and a non-judgemental approach to working with young people who may have experienced trauma or adversity.
