About the Role:Middlesbrough Council is seeking a passionate and motivated Assertive Outreach Caseworker to join our Harm Minimisation Team. The successful candidate will work within the community to support individuals experiencing substance use, domestic abuse, and homelessness. You will engage people not currently accessing mainstream services and deliver a range of psychosocial interventions to help them address issues around drugs, alcohol use, and improve their overall health and wellbeing.

This role is ideal for those who have experience in a support setting or transferable skills in caseload management, care planning, risk management, and working with vulnerable clients. Lived experience is also valued.

Key Responsibilities:

  • Engage individuals in the community who are not currently accessing mainstream services, offering support around substance use, domestic abuse, and homelessness.
  • Deliver a range of psychosocial interventions to help clients achieve their health and wellbeing goals.
  • Build strong, supportive relationships with clients to empower them to make positive changes in their lives.
  • Manage a caseload of clients, ensuring timely and appropriate interventions are in place to meet their needs.
  • Create care plans and risk assessments to support each client's unique needs and circumstances.
