Social Worker - Pathway Planning (Leaving Care)

We are seeking a dedicated Social Worker to join our team in Middlesbrough. This role involves developing personalised pathway plans for young people transitioning from the care system to independent living, providing support and advocacy, and intervening in crises to ensure access to necessary services. The ideal candidate will collaborate with various professionals to offer comprehensive support and regularly monitor and review pathway plans.

Day-to-day of the role:
  • Develop and implement personalized pathway plans for each young person, outlining their goals and the steps needed to achieve them.
  • Offer practical and emotional support, advocating for young people’s needs in areas such as education, employment, housing, and health.
  • Provide immediate support during crises, such as homelessness or mental health issues, ensuring young people have access to necessary services.
  • Work closely with other professionals, including educators, healthcare providers, and housing officers, to coordinate comprehensive support.
  • Regularly review and update pathway plans, ensuring they remain relevant and effective as young people’s needs and circumstances change.
Required Skills & Qualifications:
  • A qualification in social work and registration with the relevant social work body.
  • Experience in pathway planning and working with young people.
  • Strong crisis intervention skills and the ability to provide immediate support.
  • Excellent collaboration skills to work with a range of professionals.
  • Commitment to continuous monitoring and reviewing of pathway plans.
