Are you passionate about making a difference in the lives of individuals struggling with substance use? We are seeking a dedicated Assessment Worker to be the first point of contact for clients facing complex challenges, including substance use, domestic abuse, homelessness, and mental health difficulties.

As an Assessment Worker, you will conduct initial screenings and assessments, working closely with clients to identify their needs and connect them with appropriate support services. You will play a crucial role in ensuring that individuals receive the right care and interventions to support their journey to recovery.

• Conduct comprehensive assessments to understand clients' substance use history, mental health, and social circumstances.
• Develop tailored support plans and connect clients with appropriate services.
• Perform risk assessments and implement risk management strategies.
• Carry out proactive outreach, including home visits and community-based appointments.
• Identify and address barriers to treatment, ensuring accessibility for all clients.
• Maintain accurate records of client progress and support plans.
• Engage in reflective practice and continuous professional development.
