Job Description

  • Develop competence and deliver pharmaceutical care to patients in designated wards as part of a multidisciplinary team, ensuring safe and cost effective prescribing and administration of medicines, including contributing to safe and secure handling procedures
  • Identify, prioritise, document and action all pharmaceutical care issues for in-patient groups including drug history taking, pharmaceutical care planning, and continual monitoring of patient factors (e.g. other medical conditions, test results and response to treatment).
  • Ensure discharge prescriptions are transcribed correctly and are clinically appropriate
  • Participate in ward rounds and clinical multidisciplinary meetings where appropriate.
  • Provide advice to all grades of prescribers, other health professionals and patients on doses, possible side effects of drugs, drug interactions, therapeutic drug monitoring (TDM) services; advice on drug choice and dosages in order to optimise the treatment in a cost-effective manner
  • Ensure continuity of pharmaceutical care between care environments by liaison with primary care health professionals
