Evolve is partnering with an award-winning health and wellbeing company and Primary Care provider working within the NHS.

Our client is in search of a GPhC registered Pharmacist to join their clinical team based at a medical practice in a close-knit coastal town in Somerset (TA24). As the Pharmacist, you will be an expert advisor for medicines, repeat prescriptions, medicines optimisation and pharmaceutical care aspects for registered patients.

This is a part time, permanent position working 30 hours per week, ideally based on site but open to hybrid working if required. Opening hours are 8:30am to 5pm (with a 30-minute leeway if needed).

Ideal backgrounds for the Pharmacist:

Our client is open to considering GPhC registered Pharmacist with the following backgrounds:

  • A Primary Care Pharmacist who has completed the CPPE Primary Care Pharmacy Education Pathway.
  • A Primary Care Pharmacist who has completed the Independent Prescribing qualification.
  • Any Pharmacist background but must have completed the Independent Prescribing qualification.
  • Full UK driving licence and vehicle is required (no public transport in the area).

Role Responsibilities for the Pharmacist:

  • Provide services and support to registered patients and primary care sites, including prescribing and managing high-risk drugs.
  • Use critical thinking and communication skills to assess and address callers’ needs, considering social, cultural, and economic factors.
  • Ensure accurate prescribing and clear communication of pharmaceutical advice while identifying and managing clinical risks and protection issues.
  • Record information and advice accurately in electronic systems and access specialist databases to ensure precise guidance.
