We are seeking experienced Social Workers and Social Worker Leads to join our Hospital Discharge and Assessment Team in the Midlands. If you're passionate about making a real difference in people's lives and have a strong background in hospital or intermediate care, we want to hear from you!

Key Responsibilities:

  • Undertake Mental Health Capacity Assessments with a focus on safeguarding and individual well-being.
  • Work within a multidisciplinary team to provide holistic care for individuals in a hospital or crisis setting.
  • Offer tailored support to individuals in the Loacal Authority, empowering them to regain control of their lives.
  • Be adaptable and prepared to visit clients on-site, even on remote working days, when urgent care is needed.

Essential Requirements:

  • Experience in Hospital or Intermediate Care settings.
  • Knowledge of completing Mental Health Capacity Assessments and Safeguarding Concerns.
  • Ability to work flexibly within a multidisciplinary team.
  • Social Work qualification and registration with Social Work England.
  • Completed ASYE year or equivalent demonstrable experience in a statutory social work setting.
  • Strong knowledge of legislation, models, and theories relevant to Adult Social Work.
  • Valid UK driving license and access to a car (exceptions may apply under the Equality Act).

Working Conditions:

  • 2 days from home, 3 days in the office (with flexibility for urgent visits as required).
  • First 2 weeks will be spent in the office for training and induction.
