Reablement Support Worker

Merton Council are looking for a Reablement Support Worker to join their Team.

As a Reablement Support Worker you will:

  • Undertake introductory visits to new or re-referred service users, including undertaking environmental checks and reporting to the Reablement Officer on any relevant health and safety issues
  • Implement the reablement goal plans which enable people to achieve or regain maximum skills for independent living in the community
  • Support people to maximise their ability to manage independently by learning or re-learning the skills set out within their personal goal plan
  • Provide a person centred approach and promote independence and empowerment to support people to attain their full potential both indoors and in the community
  • Report back any problems, change in condition to the Reablement Officers and OTs

Requirements

  • Good understanding of current health and social care policies
  • Ability to undertake specialist reablement assessments
  • Experience within a similar role
