- Provide practical help and direct support to young people and young adults using the service from age 16 and are in care, or have recently left care, or who are unaccompanied asylum seeking children, to become independent and inspire them to set ambitious goals for themselves
- Manage a caseload involving the assessment, planning, implementation and evaluation of appropriate action
- Use agreed assessment frameworks in collaboration with individual to robustly assess their needs and to develop corresponding tailored intervention Pathway Plans setting out clear goals and who is going to provide assistance
- Identify accommodation needs and assist young people insecuring appropraite accommodation options
- Proactively work as an advocate for young people, navigate complex and contentious situations securing the collaboration of other agencies and individuals
- Previous experience as a leaving care personal advisor or similar
- Relevant qualification in Connexions, Youth Work, Careers, Social Services, Teaching, Health, Youth Offending/Probation Services
- Experience working with young people, including those at a risk in a variety of settings
