Reablement Officer

Merton Council are looking for a Reablement Officer to join their Team. This role will work 2 shifts and on a rolling rota.

Morning shift: 7.30am - 3.30pmEvening shift: 1pm - 9pmThis role includes weekend and bank holidays working.

As a Reablement Officer you will:

  • Support the team leader in ensuring that people are enabled to achieve or regain maximum skills for independent living in the community
  • Provide direct management support to the Support Workers
  • To provide a person centered approach and promote independence and empowerment to support people
  • To develop and ensure the service focus' on a restorative approach to maximizing an individuals independence and enable them to link back into their community

Requirements

  • Good understanding of current health and social care policies
  • Ability to undertake specialist reablement assessments
  • Ability to develop and implement goal plans and intervention plans that focus on outcomes
  • Level 2 or 3 NVQ or other form of comparable training
