JHP Recruitment are seeking an experienced Care Coordinator to join a growing team based in Newcastle-upon-Tyne and take responsibility for a major part of a domiciliary business.

Duties & Responsibilities:

As a Care Co-ordinator, you will play an integral part in managing the daily communications and care scheduling of a major part of the business. You must be personable and empathetic as you will be working with the care team to make sure all valued clients’ needs are met. This role is extremely fast paced and requires someone truly dedicated, with great organisational skills and able to remain positive and professional when working under pressure.

  • Communicate effectively and build strong, positive, working relationships with carers, clients, families, and other parties
  • Manage and roster Carer and Client schedules ensuring care plan outcomes and requests are being met
  • Ensure any changes to Client or Carer needs and availability, are communicated to all relevant parties quickly and effectively and ensure all changes are recorded appropriately
  • Accountable to the Deputy and Registered Manager, report weekly on recruitment needs, gaps in rotas, risks, Carer availability and concerns
  • Ensure outstanding compliance across your service and to support the business in achieving Outstanding feedback from CQC
  • Ensure consistent application of company policies, procedures and approved practice; and to promote the aims of the business
  • Continually review and improve processes to ensure the most effective and efficient service is being delivered
  • Responsible for a fair portion of the 'out of hours, on call’ service

The service users for this role are located in the Outer West Newcastle area (NE5, NE13, NE15 & NE20). You may need to visit staff, clients or attend events - you must hold a full and valid UK or EU/EEA driving license and have access to own vehicle for daily use.

Qualifications, Experience and Skills required:

  • Experience in a domiciliary care setting
  • Use of a scheduling / recording system
  • Good IT skills
  • Extremely well organised, excellent planning and prioritising ability with high attention to detail
