About Us:Randstad is pleased to partner with the Hospital Discharge Team at the Royal Victoria Infirmary (RVI) and Freeman Hospital in Newcastle to recruit a dedicated and compassionate Social Worker.

Job Description:

Are you a committed social worker with a passion for supporting patients during hospital discharge? In this role, you will work closely with patients, families, and healthcare professionals to ensure that discharge plans are safe, effective, and meet the needs of each individual patient.

Responsibilities:

  • Conducting comprehensive assessments of patients' needs to determine the appropriate level of care and support required post-discharge.
  • Collaborating with healthcare professionals, patients, and their families to develop and implement discharge plans.
  • Providing support, advice, and advocacy for patients and their families during the discharge process.
  • Managing and maintaining accurate and up-to-date records of assessments, care plans, and discharge arrangements.

Qualifications:

  • Qualified Social Worker registered with Social Work England.
  • Proven experience in a social work role, ideally within a hospital discharge or similar setting.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare professionals.
  • Knowledge of relevant legislation, policies, and procedures related to hospital discharge and adult social care.
