Social Worker

Long term team

Duties :

  • Regularly assess the needs of individuals to determine the level and type of support required.
  • Create detailed care plans that outline the services and support necessary to meet the long-term needs of the individual.
  • Regularly review and update care plans to ensure they continue to meet the changing needs of the individual.
  • Manage a caseload of individuals, ensuring they receive continuous and appropriate services.
  • Work with other professionals (e.g., health care providers, housing officers) to coordinate services that support the individual's well-being.
  • Ensure that the voices of individuals and their families are heard, particularly in decision-making processes.
  • Identify and assess any risks to the safety or well-being of the individual.
  • Put in place appropriate safeguarding measures to protect vulnerable individuals from abuse or neglect.
  • Report any concerns about abuse or neglect and take necessary actions in collaboration with relevant authorities.
  • Keep detailed and accurate records of assessments, care plans, reviews, and any other significant interactions.
  • Participate in regular supervision to reflect on practice, receive guidance, and manage emotional stress associated with the role.

Experience and Requirements :

  • Social Work degree
  • 2 years post qualifying experience
  • SWE registration
