Social Worker - 14+ Leaving Care Team

Location: Northumberland Council, Northumberland

Northumberland Council are seeking a dynamic and empathetic Social Worker to join our 14+ Leaving Care Team. In this role, you will provide tailored support to young people aged 14 and over who are preparing to leave the care system. Your work will involve developing and implementing personalised care plans, advocating for the needs of care leavers, and working collaboratively with various stakeholders to ensure a smooth transition to adulthood.

Key Responsibilities

  • Conduct assessments and develop comprehensive Pathway Plans for young people preparing to leave care.
  • Provide one-on-one support to care leavers, addressing their individual needs and helping them navigate challenges.
  • Collaborate with a range of professionals, including educators, healthcare providers, and housing services, to ensure holistic support for young people.
  • Advocate for the rights and needs of care leavers, ensuring they have access to appropriate services and opportunities.
  • Maintain accurate and up-to-date records in accordance with legal and regulatory requirements.
  • Participate in multi-agency meetings and contribute to the development of policies and procedures related to leaving care services.
  • Support young people in developing independent living skills, including budgeting, employment, and education.
