Are you a dedicated and compassionate Registered Nurse looking for a rewarding leadership role in the community? We are seeking a Senior Nurse Co-ordinator to join a dynamic and supportive team providing high-quality care to patients in their own homes across Newcastle Under Lyme.

About the Role

As a Senior Nurse Co-ordinator, you will play a key role in assessing, managing, and delivering care to patients, ensuring they receive safe and effective support at home. This is an opportunity to make a real difference by helping people maintain their independence, prevent unnecessary hospital admissions, and facilitate smooth hospital discharges.

Working closely with a multidisciplinary team, including Physiotherapists, Occupational Therapists, and Care Staff, you will support patients with complex needs, coordinate care plans, and provide mentorship to junior colleagues. This role offers the chance to develop leadership skills and contribute to service improvements.

Key Responsibilities

  • Assess, plan, and deliver high-quality nursing care for patients in the community.
  • Coordinate and support safe and timely hospital discharges and prevent avoidable admissions.
  • Work as part of a multidisciplinary team, collaborating with health and social care professionals.
  • Provide leadership and mentorship to junior staff and students.
  • Ensure all care delivery is evidence-based and aligns with best practices.
  • Maintain accurate patient records and contribute to clinical audits.
  • Promote independence and self-care among patients through tailored rehabilitation plans.

About You

To be successful in this role, you should be:

  • A Registered Nurse (NMC registered) with relevant post-qualification experience.
  • Passionate about community healthcare and committed to patient-centred care.
  • Experienced in managing complex cases and making independent clinical decisions.
  • A strong communicator with excellent leadership and organisational skills.
  • Able to work both independently and collaboratively within a team.
  • Willing and able to travel across the region as required.
