Job Overview:We are seeking a motivated and experienced Band 6 Community Occupational Therapist to join the Home First Team in Newcastle Under Lyme, Stoke. This role involves supporting patients in their homes to promote recovery, enhance independence, and facilitate safe, timely discharges from hospital care.
Key Responsibilities:
- Conduct comprehensive assessments of patients in their homes.
- Develop and implement personalized care plans to encourage recovery and independence.
- Work collaboratively with multidisciplinary teams to ensure holistic care.
- Provide advice and equipment to improve living environments for safety and functionality.
- Support discharge planning and prevent unnecessary hospital admissions.
- Maintain accurate and professional records of patient care.
Requirements:
- Registered Occupational Therapist with HCPC registration.
- Proven experience in community-based occupational therapy.
- Strong communication and problem-solving skills.
- Ability to work independently and manage a diverse caseload.
- Essential: Full driving license and access to your own vehicle.
- Flexibility to work shifts across 7 days.
