Job Overview:

We are seeking a skilled and motivated Locum Band 6 Occupational Therapist to join a dynamic team in Newport, Wales. This is an excellent opportunity for an experienced OT to work on a winter ward within a Discharge to Assess (D2A) setting, supporting patients transitioning from hospital to home or other care settings.

Key Responsibilities:
  • Provide high-quality occupational therapy assessments and interventions to facilitate timely and safe discharges.
  • Work within a Discharge to Assess (D2A) model, supporting patients in regaining independence.
  • Collaborate with multidisciplinary teams, including nurses, physiotherapists, social workers, and community services.
  • Develop personalized care and rehabilitation plans to enhance patient recovery.
  • Ensure effective discharge planning, signposting patients to community resources as needed.
  • Maintain up-to-date and accurate documentation in line with NHS policies and procedures.
Requirements:
  • HCPC-registered Occupational Therapist.
  • Previous experience in hospital-based rehabilitation, acute care, or D2A settings preferred.
  • Strong clinical reasoning and assessment skills.
