The main role:

To provide high quality support to teams within adult services.

  1. To manage and respond to enquiries into adult services.

  2. To provide people with information and advice in relation to care and support and assistance in accessing care and support.

  1. To respond to enquiries in an informative and knowledgeable way to enable people to better help themselves and others.

  1. To explore options with individuals for meeting their care and support needs and to empower the individual to access early intervention and preventatives services.

  1. To undertake proportionate assessments of care and support needs when offering advice and assistance.

  1. To inform senior colleagues of any concerns which arise in relation to the safety of vulnerable adults.

  1. To work in partnership with individuals, carers, service providers, voluntary sector and other professionals.

  1. To perform all the functions of the post consistent with current legislation and within the policies standards and procedures established by the City Council.

  1. To take responsibility for all the functions of the post within the boundaries of delegated responsibility

  1. To provide a quality standard of report and to maintain accurate case recording using the appropriate software as required

  1. To attend team meetings and undertake any necessary and appropriate team duties and activities.

  2. To provide support and assistance to Social Workers and other colleagues.
