Newport County Council is looking for an experienced social work assistant to join the Home First team within Adult's services.
The position is temporary and full-time for an initial period of 3 months, with a pay rate of £16.92 per hour.
Candidates should have experience in Health and Social Care, including conducting care and support assessments.
An enhanced DBS check and a full UK driving license are also required for this role.
The role involves managing a caseload and providing information and support to individuals regarding their care needs.

Newport County Council are seeking to recruit an experienced social work assistant based in the Home First team within Adult’s services. You will manager a caseload of people, each requiring customised care and support plans. The role is a temporary, full time position for an initial period of 3 months with a view to this being extended on a rolling basis and is offering a pay rate of £16.92 per hour.The duties of this role will include:

To provide people with information and advice in relation to care and support and assistance in accessing care and support.
To explore options with individuals for meeting their care and support needs and to empower the individual to access early intervention and preventatives services.
To respond to enquiries in an informative and knowledgeable way to enable people to better help themselves and others.
To undertake proportionate assessments of care and support needs when offering advice and assistance.

The ideal candidate will have:

Enhanced DBS check
Experience of working within Health and Social Care
Experience of carrying out care and support assessments
Full UK driving licence
Be able to work upon own initiative and within a team
