• Thorough patient assessment
• Individualized care plan development
• Interdisciplinary care coordination

• Package of care oversight

• Collaboration with interdisciplinary teams
• Data analysis and trend identification
• Accurate record keeping and documentation

• Knowledge of latest clinical guidelines

This role requires a dedicated and skilled individual with a strong commitment to patient-centered care. We are seeking candidates who possess:

• A current NMC PIN
• A nursing qualification in Learning Disabilities or Children's Nursing (or both)
