Contract: Fixed-term, maternity cover 11 months
Hours: Full-time preferred, flexibility available (minimum 33 hours p/week)
Location: Fully remote working, although ad-hoc visits to their practices in Northamptonshire especially for training/shadowing at the start of employment.

Responsibilities:
- Conduct Structured Medication Reviews and support Ageing Well MDTs.
- Provide dedicated medication reviews for care home patients and chronic conditions.
- Oversee medicines safety and assist with quality improvement initiatives.
- Collaborate with hospital and community pharmacies to optimize care continuity.
- Engage in the Prescribing Incentive Scheme and lead Eclipse monitoring.
About You:
- Qualifications: MPharm (or equivalent), GPhC registration, and although prescribing is not essential you will need to have completed CPPE training.
- Experience: Primary care with strong clinical and therapeutic knowledge.
- Skills: Strong interpersonal and IT skills, adept in MS Office; SystmOne experience beneficial.
