Responsibilities:

  • Conduct Structured Medication Reviews and support Ageing Well MDTs.
  • Provide dedicated medication reviews for care home patients and chronic conditions.
  • Oversee medicines safety and assist with quality improvement initiatives.
  • Collaborate with hospital and community pharmacies to optimize care continuity.
  • Engage in the Prescribing Incentive Scheme and lead Eclipse monitoring.

About You:

  • Qualifications: MPharm (or equivalent), GPhC registration, and although prescribing is not essential you will need to have completed CPPE training.
  • Experience: Primary care with strong clinical and therapeutic knowledge.
  • Skills: Strong interpersonal and IT skills, adept in MS Office; SystmOne experience beneficial.
