About the Role:

  • We are looking for a skilled and experienced Band 6 Community/HEF Dietitian to join our team on a locum basis for a 5-month contract. This exciting role involves providing high-quality dietetic care through home visits and interventions in nursing homes (NH), with a focus on health and food (HEF) services for a diverse range of clients in the community.

As a locum dietitian, you will be working directly with patients, ensuring nutritional support and care are tailored to individual needs, particularly in community and nursing home settings. You will also be part of a multidisciplinary team working to improve the health and wellbeing of patients in the local area.

Key Responsibilities:

  • Deliver dietetic assessments, care plans, and interventions in the community and nursing homes, with a focus on health and food (HEF) services.
  • Provide expert advice on nutrition and dietary management for patients with complex medical conditions.
  • Work independently to manage a caseload of home visit patients and care home interventions.
  • Liaise with healthcare professionals and carers to ensure the best care is provided to each patient.
  • Monitor and review the progress of patients and adjust interventions as necessary.
