We are seeking an experienced Social Worker to join our Hospital Discharge & Assessment Team. This is a fast-paced role, requiring expertise in hospital discharge planning, strengths-based assessments, and care planning. As a Social Worker you will play a crucial role in supporting patients as they transition from hospital to appropriate care settings.

Key Responsibilities:

  • Conduct Strengths-Based Assessments and develop person-centred care plans.
  • Support and lead Best Interest Meetings and Mental Capacity Assessments.
  • Facilitate safe and timely hospital discharges while ensuring compliance with relevant legislation.
  • Work closely with multidisciplinary teams, including health professionals and external agencies.
  • Support safeguarding processes where required.
