We are looking for a dedicated and compassionate RNLD or RMN to join a S117 Joint Funding project, supporting a client in the North West. This role is hybrid, you'll need to be available for face to face visits.
Day to Day Duties:
- Deliver care plans and interventions.
- Collaborate with multi-disciplinary teams, including social workers and healthcare professionals.
- Monitor and assess client progress, ensuring their health and well-being.
- Provide support with activities of daily living, promoting independence and choice.
- Conduct risk assessments and manage care-related documentation.
