Social Worker - Hospital Discharge Assessment Team

Up to £32 per hour

Location:Kings Mill Hospital, Sutton-in-Ashfield

About the Role:

We are seeking a skilled and experienced Social Worker to join our Hospital Discharge Assessment Team. You will play a crucial role in facilitating timely and safe discharges from Kings Mill Hospital.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' social and care needs upon hospital admission.
  • Develop and implement discharge plans, coordinating with multidisciplinary teams to ensure seamless transitions of care.
  • Liaise with community services, care providers, and other relevant agencies to secure appropriate post-discharge support.
  • Advocate for patients' needs and rights, ensuring they receive the best possible care.
  • Participate in a rota system to cover the Transfer of Care Hub at Kings Mill Hospital.
  • Maintain accurate and up-to-date records.
  • Promote effective communication and collaboration with healthcare professionals, patients, and families.

Essential Requirements:

  • Social Work Degree (or equivalent)
  • HCPC Registration
  • Significant experience in a hospital or community setting
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work shifts, including weekends and evenings

Desirable Requirements:

  • Experience in discharge planning and complex case management
  • Knowledge of local health and social care services
  • Ability to manage a demanding workload
