Social Worker - Hospital Discharge Assessment Team
Up to £32 per hour

About the Role:
We are seeking a skilled and experienced Social Worker to join our Hospital Discharge Assessment Team. You will play a crucial role in facilitating timely and safe discharges from Kings Mill Hospital.
Key Responsibilities:
- Conduct comprehensive assessments of patients' social and care needs upon hospital admission.
- Develop and implement discharge plans, coordinating with multidisciplinary teams to ensure seamless transitions of care.
- Liaise with community services, care providers, and other relevant agencies to secure appropriate post-discharge support.
- Advocate for patients' needs and rights, ensuring they receive the best possible care.
- Participate in a rota system to cover the Transfer of Care Hub at Kings Mill Hospital.
- Maintain accurate and up-to-date records.
- Promote effective communication and collaboration with healthcare professionals, patients, and families.
Essential Requirements:
- Social Work Degree (or equivalent)
- HCPC Registration
- Significant experience in a hospital or community setting
- Strong assessment and care planning skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work shifts, including weekends and evenings
Desirable Requirements:
- Experience in discharge planning and complex case management
- Knowledge of local health and social care services
- Ability to manage a demanding workload
