Job Description:

We are seeking an Occupational Therapist to join the community therapy team, working across Helensburgh. This role focuses on community and hospital discharge work, helping to facilitate smooth transitions for patients returning home or to the community after hospital care.

Key Responsibilities:

  • Conduct assessments and provide interventions for patients transitioning from hospital to the community.
  • Collaborate with multidisciplinary teams to ensure the best possible care and discharge outcomes.
  • Manage a caseload of patients, ensuring timely and effective occupational therapy services.

Requirements:

  • Must hold a valid OT qualification and be registered with HCPC.
  • Driving is essential, and access to a car is preferred (though a pool car may be available).
  • Previous experience in community-based OT or hospital discharge roles is desirable.
