- Clinical Care: Chronic disease management (e.g., asthma, COPD, diabetes), wound care, immunisations, cervical screening, and phlebotomy.
- Health Promotion: Delivering lifestyle advice, sexual health education, and supporting public health initiatives.
- Patient Support: Planning and reviewing treatment with patients and colleagues, supporting long-term condition clinics.
- Home Visits: Participating in community outreach and home-based patient care.
- Collaboration: Working alongside GPs, HCAs, pharmacists, and trainee clinicians to ensure consistent, high-quality patient outcomes.
- Administration: Using computer systems ffectively for clinical documentation and data management.
- Registered General Nurse with full NMC registration
- Experience in a GP Practice / Primary Care setting
- Competency in chronic disease management, wound care, immunisation, cervical screening
- Proficient in EMIS systems
- Excellent communication, teamwork, and organisational skills
- Qualifications in cervical screening, immunisations, and disease management
- Experience in student supervision and quality improvement initiatives
