What you will do:

  • Assess, plan, and implement care plans for patients transitioning between hospital and home or alternative care settings.
  • Work collaboratively with the healthcare team to ensure seamless care is provided to patients.
  • Conduct assessments to understand the social, emotional, and environmental needs of individuals.
  • Provide advice, support, and resources to patients and their families/carers to aid recovery and rehabilitation.
  • Act as an advocate for patients in multi-disciplinary meetings to ensure their needs are effectively communicated and met.
  • Maintain accurate and up-to-date case records and reports in accordance with council policies.
