Responsibilities
  • Develop and implement comprehensive care plans tailored to individual needs.
  • Conduct thorough and regular assessments, accurately monitoring progress and adapting plans as needed.
  • Provide empathetic support and guidance to individuals and their families, fostering a collaborative approach to care.
  • Effectively liaise with a wide range of health care professionals and agencies to ensure seamless care coordination.
  • Maintain meticulous and accurate records and documentation, adhering to all relevant policies and procedures.
  • Actively contribute to a positive and supportive team environment, sharing expertise and fostering collaborative working.
Requirements
  • NMC registration
  • DBS on the update service
  • Proven experience in mental health, ideally within a Community setting ideally within the NHS
  • Excellent communication, interpersonal, and organisational skills are essential
  • Ability to work both independently and as part of a multidisciplinary team
  • A strong commitment to delivering high-quality, person-centred care
