Your Key Responsibilities:

  • Mental Health Support: Provide expert care to individuals facing moderate to severe mental health challenges.
  • Collaborative Work: Partner with Oldham's CMHT to deliver mental health support to the local community.
  • Care Act Compliance: Complete statutory responsibilities, including assessments, funding applications, COPDOLs, 21a challenges, and more.
  • Transition Support: Help young people transitioning from children services to adult mental health services.
  • Care Coordination: Oversee and manage personalised care and treatment packages for a caseload of service users within the secondary mental health service.


Requirements:

  • Social Work England Registration: Current and valid.
  • Qualification: Social Work Degree or equivalent qualification.
  • Enhanced DBS: Required for this role.


Experience:

  • Adult's Services: Prior experience in adult social care or mental health settings is essential.
