Job Description:

  • Undertake assessments of Adults and Carers in line with legislation, policy and procedures.
  • To provide practical support and information for Adults and Carers.
  • Work with individuals to develop support plans by arranging and monitoring the delivery of services/payments to Adults and Carers.
  • To work with your team colleagues and other professional staff to ensure that service user’s needs are met.
  • Monitor, review services to ensure that they are provided effectively and in accordance with need.
  • Prepare individual support plans reflecting the assessment of needs of the Individual and their Carers.
  • Offer Choice and Control by promoting Direct Payments.
  • Work creatively to achieve best value by providing cost-effective services.
  • Monitor and review support plans and the effectiveness of services provided for Adults and Carers.
  • Provide support to qualified staff to assist with tasks identified as requiring practical and on-going input.
  • To maintain Adult and Carer records in accordance with the departmental policy and practice.
  • To engage with users and carers in such a way that is consistent with anti-discriminatory practice.
  • To work in partnership with partners and 3rd party organisations.
  • To develop and maintain knowledge skills and expertise in assessment, planning and review.
  • To support colleagues with preliminary enquiries for safeguarding.
  • Participate in the operation of duty and respond accordingly to need.
  • To work across teams within the Division as and when needed.
  • Contribute to service development by providing information and ideas for future services.
