- HCPC or NMC qualification is mandatory along with a full driving licence.

- Vast knowledge, of Acquired brain injury, Brain injury, in adult care, with a variety of other condition knowledge, including Spinal injury, Amputations and paediatrics.
- You must have worked with patients in a rehabilitation/complex care setting. -
- Proven experience as a Case Manager or in a related clinical role is extremely advantageous.

- Conduct thorough assessments of patient needs and monitor their progress throughout the rehabilitation process.
- Complete immediate needs assessments and recommendations of care
- Liaise with external healthcare providers and insurance companies to coordinate and facilitate patient care.
- Provide ongoing support, education, and counselling to patients and their families.
- Maintain accurate and up-to-date patient records and documentation.
