  • RESPONSIBILITIES
  • We provide community enabling and short term accommodation to people who's long term goals are to move onto eventual independent supported living or achieve higher independence.
  • You are responsible for promoting outcomes for peoples lives.
  • Outcomes are specific and keep the person at the center of everything!
  • You will be required to undertake some element of lone working in the community, from peoples own homes or at our accommodation.
  • Responsibilities for support include;
  • Building better natural circles of support and friendships, exercise and staying healthy, food and diet, activities of daily living, managing and maintaining a safe and habitable home, budgeting and money skills, making use of local services and facilities, opportunities for volunteering an employment, making use of public transport and travel training, and more.
  • No personal care requirement
  • It is CLIP's aim to reduce the need for paid support over an agreed and realistic time putting greater emphasis on building better links with our local community, bespoke friendships with others and the encouragement of strength based support towards peoples futures.
  • You will effectively work with the goal to eventually make yourself as professionally redundant as possible with any given person over the course of time. We need people with a genuine desire to make change to the lives of vulnerable others, to deliver support in such a way that doesn't nurture over dependency, keeps peoples lives feeling natural, encouraged and inspired, free from over bias or social institution and strives for a push away from conventional services.

  • You will work in relation to the plan of outcomes specified and led by the people you support, with the guidance of your operations manager, community mapping and planning support team and the wider health and social care team Torbay.
  • With continued guidance and support from CLIP you will be responsible for your own case load alongside fellow colleagues, ensuring consistency of care and support and meeting outcomes.
