Our client, a leading organisation in supported housing, is seeking a dedicated Support Worker to join their Complex Needs team.

About the Role:

As a Support Worker, you will play a crucial role in empowering individuals at risk of homelessness to rebuild their lives and work towards independent living. Working within a small, supportive team and guided by a Team Leader and Team Manager, you will provide high-quality, person-centred support to a small caseload of customers.

Key Responsibilities:
  • Develop, review, and implement personalised support plans and risk assessments.
  • Adopt a holistic approach to support, assisting individuals in achieving personal goals.
  • Collaborate with external agencies to address specific needs, including mental health, substance misuse, and financial support.
  • Guide customers in exploring education, training, and employment opportunities.
  • Help individuals maintain tenancies and prevent homelessness through proactive support.
  • Manage risk effectively and take a solution-focused approach to challenges.
About You:

To be successful in this role, you will need:

  • A strong understanding of support planning and risk management.
  • Experience working with individuals with complex needs.
  • Excellent communication skills and the ability to build meaningful relationships.
  • Strong IT and record-keeping skills.
  • Confidence in lone working as well as being a proactive team player.
  • Adaptability, resilience, and a solution-focused mindset.
  • A flexible approach to work and the ability to manage pressures effectively.
  • This role requires an enhanced DBS check.
