Job Title: Band 6 Neuro Occupational Therapist - Integrated Stroke Early Supported Discharge (ESD) and Community Neuro Rehabilitation (CNRS) Service (Full Time, Locum)

Position Overview:

We are seeking a dedicated Band 6 Neuro Occupational Therapist to join our Integrated Stroke Early Supported Discharge (ESD) and Community Neuro Rehabilitation (CNRS) Service on a locum basis. This full-time position offers the opportunity to work with patients recovering from strokes and other neurological conditions, providing essential rehabilitation and support in the community.

Key Responsibilities:

  • Conduct comprehensive assessments of patients to identify their rehabilitation needs and functional abilities.
  • Develop and implement individualized therapy plans aimed at maximizing independence and quality of life.
  • Provide therapy interventions in patients' homes and community settings, focusing on Activities of Daily Living (ADLs) and functional skills.
  • Collaborate with multidisciplinary team members, including physiotherapists, speech and language therapists, and nursing staff, to ensure coordinated care.
  • Educate and support patients and their families in managing the effects of neurological conditions.
  • Monitor and evaluate patient progress, making adjustments to treatment plans as necessary.
  • Maintain accurate documentation of assessments, interventions, and outcomes in line with service standards.

Qualifications:

  • A recognized degree in Occupational Therapy.
  • HCPC registration is essential.
  • Experience in neuro rehabilitation, particularly with stroke patients, is highly desirable.
  • Previous community-based experience is preferred.
  • Strong communication and interpersonal skills, with a commitment to providing patient-centered care.
  • Ability to work independently and as part of a multidisciplinary team.
