  • Contribute to the assessment of clients' needs, providing input into the creation, development and review of care plans under supervision involving other service providers as required and ensuring the involvement of clients in planning and evaluating their own care.
  • Deliver care and support for clients as required to ensure clients' personal hygiene, dietary needs and medication needs are met whilst ensuring their privacy, dignity and independence is maintained and positively respected at all times and in accordance with agreed care plans.

  • Provide direct care and support to individuals in line with their needs and care plans.
  • Utilize CPI (MAPA) techniques to manage and de-escalate potentially challenging situations.
  • Collaborate with other team members to ensure consistent and high-quality support.
  • Maintain accurate and up-to-date records of care and incidents as required.
  • Adhere to all safety guidelines, policies, and procedures to ensure a safe environment.
