Description

* Working as part of the wider MDT

* Supporting the At Home and Assisted Discharge Team

* Supporting services users who are being discharged from hospital into their own homes

* Complete and Follow Support Plans

* Provide practical and emotional support

* Based from one site in PE3 with visits in the Peterborough area to support service users in their own home

Profile

* Experienced as a Support Worker

* Can work as part of a team but also be confident working alone

* Basic IT Skills

* CAR DRIVER IS ESSENTIAL FOR THIS ROLE

* Client will arrange for you to complete their mandatory training

* This role is subject to an Enhanced Adult DBS (which can be applied for if required)

Job Offer

* Monday to Friday 10am - 6pm

* Occasional weekends

* 6 Week Contract which may be extended
