Salary £29,406 per annum (includes on call)

- Senior Care Coordinator – Daily duties

Job Summary
The Care Coordinator is responsible for coordinating the provision of care and support staff to service users, typically within their homes or the local community. Key responsibilities include arranging and communicating staffing resources, managing referrals, conducting interviews, and inducting new starters. The role also involves performing care and support work during emergencies and on-call duty.
Role, Responsibilities, Behaviours & Values:
- Coordinate care planning with a focus on person-centred care for adults.
- Strive to deliver services beyond the basic expectations of the service user care plan.
- Monitor care worker and service user progress, ensuring care and support plans are updated through quality monitoring systems and team supervision.
- Alert the Manager regarding any concerns about care quality or safety risks.

Relationship Management:
- Maintain positive relationships with commissioners, healthcare professionals, and external agencies.
- Report concerns to social workers, the registered manager, and external professionals/agencies.
Customer Service:
- Maintain professional communication with staff, service users, families, and other agencies.
- Handle complaints in accordance with company policy.
- Coordinate new referrals, risk assessments, and care planning.
Compliance and Monitoring:
- Ensure compliance with LA contracts, the Care Act 2015, CQC regulations, and company policies.
- Conduct risk assessments, care plan reviews, and quality checks of service users.
- Conduct appraisals and supervise the care and support worker team.
- Handle staff discipline, grievances, complaints, and absence management according to company policy.
Health and Safety
As an employee of the Care Coordinator has a duty under the Health and Safety at Work Act 1974 to:
- Take reasonable care of their own health and safety and that of others affected by their actions or omissions.
- Cooperate with the employer to ensure compliance with health and safety legislation.

Essential:
- Commitment to complete Diploma Level 3 in Social Care.
- Complete Palliative Care, Team Leader, and Children’s
- Care Modules.
