Position Overview

We are seeking a dedicated and experienced Band 6 Social Worker to join our team. The role involves providing comprehensive social work services within [specific setting, e.g., healthcare, mental health, child protection, or community services], ensuring the well-being and safety of individuals, families, and communities. This position requires advanced practice knowledge, assessment skills, and a commitment to multidisciplinary collaboration.

Key Responsibilities
  1. Assessment and Care Planning:

    • Undertake comprehensive assessments of individuals and families, considering their needs, risks, and strengths.
    • Develop and implement care and support plans tailored to the needs of clients, in accordance with relevant policies and legislation.
    • Conduct safeguarding investigations and risk assessments to ensure client safety.
  2. Case Management:

    • Manage a varied and complex caseload with appropriate prioritization.
    • Monitor and review care plans to ensure effectiveness and adapt them as required.
    • Provide crisis intervention and support for individuals and families in challenging situations.
  3. Collaboration:

    • Work closely with other professionals, including healthcare providers, housing teams, and legal services, to deliver coordinated care.
    • Actively participate in multidisciplinary meetings, case conferences, and strategy discussions.

- Min 1 year experience- Need to have completed court of protection actions- Need to have a understanding of CHC framework- Need to have worked in MDT, (Multi-Disciplinary Team)
